Using this in a Cisco SG500X switch in a data center. The part came with all genuine looking labels and packaging. It was marked SFP-10G-LR-X, which turns out to be an extended temp range part.
It took experimentation to figure out how to insert the module and then to insert the cable, since I don't use optical connections that often. To insert the module, flip down the wire bail first. Insert all the way in (if it doesn't go all the way, it's probably upside down), then remove the rubber dust plug and flip the wire bail up. Remove the dust plugs from the yellow optical cable with blue connector and insert into the module until the latch engages. If the latch doesn't engage, the cable is probably upside down.
Everything started working immediately.
